Fully trained immunisation teams will visit schools to provide these free vaccines.

Your child will only be vaccinated if you provide consent.
Complete the online consent form to ensure your child receives their immunisations this school year.

Hard copy consent forms are available from your child’s school office. Complete and return them ASAP! See below for download link.

💡𝗔𝗱𝗱𝗶𝘁𝗶𝗼𝗻𝗮𝗹 𝗜𝗻𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻:
Missed a vaccine at school? Visit your GP, community immunisation clinic, Aboriginal medical service, or participating pharmacy for a catch-up vaccine.
For children under the care of the Department of Communities, contact your dependent’s case worker to complete the form.
