Fully trained immunisation teams will visit schools to provide these free vaccines.

Your child will only be vaccinated if you provide consent.

💡𝗔𝗱𝗱𝗶𝘁𝗶𝗼𝗻𝗮𝗹 𝗜𝗻𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻:

Missed a vaccine at school? Visit your GP, community immunisation clinic, Aboriginal medical service, or participating pharmacy for a catch-up vaccine.

For children under the care of the Department of Communities, contact your dependent’s case worker to complete the form.
